This has happened to me also, so far only with one user. We are upgrading from various versions of 5, mostly 5.8 or higher. We had a special bookmark template for R5 with our corporate welcome page but no other changes.

We have upgraded 5-8 other users already and no one else had this problem but they all had our special bookmark template.
